Child Care Resources, Handbooks, and Forms

The Office of Child Care has a host of helpful handbooks and forms for child care providers after they apply to operate in Rhode Island. Some are resources related to licensing while others are specific for providers who serve children enrolled in our Starting RIght Child Care Assistance Program (CCAP). The CCAP-specific resources below are in addition to the financial information found here.

In addition to the resources you will find on this page, the Office of Child Care is making available the emergency preparedness plan to assist providers in responding to an emergency or disaster.

Monitoring Reports

Child Care Licensing is federally required to monitor licensed child care centers and family child care programs. A blank copy of the monitoring checklists are below. For non-compliances for individual centers or family child care programs, please search using the provider portal here.

If you need a copy of a monitoring report in a language other than English, please contact the Office of Child Care’s licensing team at DHS.Childcarelicensing@dhs.ri.gov
